All-time favourite game....wow. That's hard because I love the classics and the new games.
For classics, I'd have to say Metal Slug (all of them)
For recent, Unreal Tournament 2004. This game was a breakthrough in multiplayer FPS!
Classics take the cake for great fast-paced basic gameplay.
Newer games are the icing, adding deeper graphics and complexity to the gameplay.
In my opinion, you need both ingredients to make that legendary cake!!

Depends on what era I was in at the time. I've probably logged the most hours on Battlefield Vietnam for the PC, but also RBI Baseball for NES, College Football II for Sega Genesis, UFO! for Odyssey II and others.
As far as being a pure videogame, even though it's simple, UFO! may be the best videogame of all time as far as movement and addiction goes.

I'd have to say Master Of Orion (the original one) or Angband.
