Hard to say. Some questions you should ask yourself are: What are you feeding him? Does he have a heat lamp/stone? Does he have the proper uv lamp? What bedding does he have? Crushed walnut and sandy bedding can get in his food and obstruct him. How old is he? Finally, make sure that his cage is clean. Oh, did you change his food recently?

Most likely from improper living conditions. Here is a good care sheet website--make sure your heat, UVB lighting, humidity and diet are correct. If not, they will quickly become ill and die.

if his eyes are shrunk in, it is dehydrated most likely, you should mist its cage daily in the moring to let him lick the "dew" from the cage.
