There are sevices out there like the link below but I believe if you search the county you were born in ther will be info on ow to obtain one. This is what I did and I also got three copies so I could put one in a safe place.
As for you SSN card you can just go dow to the local SSN office and get a new card. Check with them to see what info you need to provide. Birth cert should do.

Most states you can now go to the website for that state and order a copy of your birth certificate. You can have the service expidited so you can get it quicker. All you should have to do is type in the state's name, then .gov, and it should take you to the web site. Look for the Department of Human Resources or something like that, and it will tell you what you need to do to get it and have it sent to you.
As for the social security card, all you have to have is a valid ID from any state and they can issue you a replacement card. If you don't have a SS card, then you will need your birth certificate, insurance card, or some other form of ID, school id, govt. id, there are lots of choices. You can go to any office to have that done. You can also go on the web to find the nearest office to you by going to socialsecurity.gov, or .org, not sure which one....Oh yeah, marriage or divorce papers work also if they are certified.

Hope this all helps and good luck to you!!!

Write to the county court house where you were born and request your birth certificate. Go to a Social Security Office to get a copy of your social security card.

do you have a passport/expired ID from the old state you lived in? along with utility bills?

if so, that should be good enough to get a birth certificate. all you need to do is contact city hall. so call the town you were born in and talk to city hall or the doctors office of the hospital you were born in. they can trace their records
