who incurs the postage fees?

The person who you send the letter to gets charged and extra for delivery.

they put a letter through the recipients door telling them they have a letter with not enough postage on it, waiting at the depot. So when they go and pick it up, not only do they have to pay the postage but they add £1 for handling fees.

If the post person doesn't notice that it has insufficient postage and there is insufficient information to return it to you, then it will be thrown out. The US postal service throws at a lot of mail every year. Some mail, even if there is sufficient information to return it, will be thrown out as well. For instance, if you send mail to Santa at the North Pole, they won't return it (lest you find out that Santa doesn't exist). Instead it is thrown away and he apparently ignores you! Of course, clever parents intercept Santa-bound mail so that they can know what little Johnny wants for Christmas. By the way, the "Dead Letter" office is no longer. There is simply too much undeliverable mail and not enough funding to support it. Its too bad - there are some great stories there.

If there is a return address...they'll send it back to YOU unless it looks like unimportant rubbish...then they will just toss it.
If insufficient postage, they may give you a paper to rediem the letter and pay the few cents postage. In the US...UK rules may differ!

It will go to the local post office who you are sending the letter to and they will get a card to say they have a letter waiting with no postage paid and they will have to pay it.

The recipients will be charged for postage at the other end! Embarrassing isn't it! I once got a sticker thing on a letter saying the sender had not paid enough but they had delivered it this time so please could I inform them in future.

sometimes it comes back to you,
sometimes it gets to the recipient with a postage due request,
and sometimes it goes in the dead letter file which i understand is this HUGE warehouse (prob several of them) where you will never see it again.
How each option is decided i dont know. its the post office !
since all the employees are affirmative action hirees, its anyones guess -they do what they want since none of them are qualified

normally who ever received the letter will have to pay however, i sent a letter the other day and forgot to put a stamp on and it was delivered without charge!
