A cover letter never hurts. In it you can tell exactly why you want the job and fill in any details of previous experience that you can't fit on the application, but may help you get the job.
Under no circumstances make it longer than one page, however.

Don't include your references on your resume; the extra space saved will be useful to list down your other skills and achievements. Just place a note in your resume "References available upon request" This list is a separate document you must prepare, so that when an employer asks for one you would be able to send one immediately. Keep this list updated; call them up yourself to see if their number is still their current contact number. An unreachable number on your resume reference list will hurt your chances of getting hired; most hiring managers will treat this as a sign of negligence on your part.

Yes, but keep it short and sweet. Fill out the application neatly and add a couple of references to the resume. That way you can say, "I've attached my resume as well as references." Even if it does not matter to them, it'll show you how it's done right. By the way, a good cover letter can often add to your starting wage right off. Keep that in mind in a couple of years.

As someone who hires lots of people, I can tell you that cover letters and resumes are always very much appreciated and will give you a leg up on the job. In addition, when you apply for the job, be sure to come in after lunch but before dinner (around 3pm), bring your OWN pen, and dress like you are applying for a job at a bank. Good luck!

By all means YES! Your sincere interest and extra effort will help a lot.
Before you get started, go to their website and learn a bit about the company, and eat there few times and observe all you can. That way you can talk intelligently with the interviewer.
