I scared the heck out of myself when I was 10 and I still won't go in one till this day.

Busy intersections on wide streets. I was once trapped by traffic on a safety island for 10 minutes, and ever since I've had a phobia about having a dizzy spell while crossing a major boulevard and being run over.
Sometimes I have to go to a light at a smaller cross street, but mostly I just suck it up and cross as fast as I can.
