While I can't answer with a 100% assurance, I have seen in the past where players that get hurt playing for there team, the teams will ensure player is payed and medical bills paid for (Through players insurance most likely). However players that are injured outside of play for their team are usually not paid and have only their health insurance to cover them. Prime example is Byron Dafoe, when he was with the Atlanta Thrashers, was injured by slipping on the ice outide of the team's hotel in Toronto (Ironic, I know). he was pretty much dropped immedietly by the Thrashers, and I don't think he's played since.

Some teams do that. I'm pretty sure the Oilers paid Pisani his salary while he was out with his illness.
The Sabres didn't though. Teppo has been out all season because of heart surgery. Instead of paying Teppo's salary while he is out, the Sabres suspended him for not passing his physical and he isn't getting paid

Yes presuming they got injured during play. The only time they don't get paid is if they're suspended.
If they got hurt outside of hockey I'm not sure.

Yep. It's the same thing as if you get injured on the job, your employer has to pay you for your recovery time. Alex Mogilny was one of the only players getting paid during the lockout because he injured his hip in the previous season.

A lot of players also have incentives built into their contracts for the playoffs, such as they score X goals they get 1 million type of thing. Mostly it comes from the lump sum the team gets for making the playoffs. If I remember correctly for winning the Cup players make almost 1 million in bonus.
