I remember my phone number, my mother's phone number, my father's phone number, my girlfriend's phone number and her parents' phone numbers. Plus, some of these are international so I must know the numbers to use in front of them. I use calling cards as well, and I know all of these numbers in my mind without looking.

About 5. I always have my cell phone on me so it's no big deal. I can remember my cell number but not my home number. lol. It's not like anyone would answer my house phone anyway
