Number 1. I think one is supposed to inhale through the nostrils while closing the mouth then exhale slowly from the mouth.

The nose helps to adjust the incoming air for the lungs, it contain hair to prevent particles of any type,including dust and insects...., from getting inside, it also contain turbenates and sinuses ( things to increase the surface area of the lining of the nose) thus more effective in regulating Temperature and humidity of the incoming air, and let us not forget the "nature" given sneezing (protective mechanism).....plus some other functions( yes smell and else).
So, The natural healthy way is No. (1)
inhale=nose, exhale=mouth

Now that you asked me, I'm not even sure how I breathe. I need to look at how I breathe to know.

Always inhale from nose and exhale from nose and that too take deep breath from your stomach and not from your chest which are shallow breaths.

Most of the time I do #4, mostly out of habbit because when I was younger I had major sinus problems and could only breath out of my mouth. Plus, I feel like I get more air through my mouth.

#2 90% of the time, unless I'm winded then it's 4.
If I'm in pain then definitely #1
I don't think I do #3 at all.
