The Omen (original)~metal pole skewers priest after having been struck by lightning
Final Conflict: Omen II~elevator cables sheer through car, cutting Meshach Taylor in half at the waist~Yuck!
Sometimes They Come Back...Again~I think one person gets killed when playing cards are thrown by the back-again baddie.
Theater of Blood~Robert Morley's character is fed to death with a pastry pouch (the only name I heard it called when I worked at a restaurant though surely there is a specific name).
Hellraiser III: Hell on Earth~I think this is the one in which Pinhead(?) kills someone with CDs, rather like the playing cards are used. I'm not sure if one copied the other.
Ghost in the Machine~Is this the first death by open microwave?
Gremlins~mother microwaves a gremlin
Trilogy of Terror 2~I think that the fetish doll is microwaved in this one, so perhaps that's not such an unusual way of death.
Final Destination 2~sliding ladder on fire escape

I may sound cliche saying this but I still love in one of Friday the 13ths when Jason scoops up the girl in the sleeping bag and slams her into a tree. One of my personal favorites. Also not a horror film and not shown in the film but in clerks, Randall talks about how his cousin died by breaking his neck trying to suck his own di*k. Thats an unusual death.

Cannibalism, some sort of paranormal phenomenon that the person dies from shock. A demon/ghost entering the body, thus the person becoming possessed, and then ends up dying in some unconventional way
