I don't think this is possible. Even filtered internet can let things through. I would make an account for her that is separate from your account (do this in the control panel) then password protect your account. That way, she cannot add things to your history.

Best way - don't let her use the computer.
I've tried several software packages and none of them have been able to block 100%. That's really too bad. Those packages with black-lists just can't keep up with the new sites.
So, maybe you should enable a filter which would only allow her to get to "approved" sites? Firewalls have this capability and I think Mozilla does too.

The only way to block all porn sites is to allow only specific sites and block all others. This is much more difficult than it sounds, because, if you're like most users, you're constantly going to new sites, but it is a possibility.
The next best thing is to use block lists. This list will always be incomplete, and there are ways around this. This is the most practical approach, though.
Both of these types of programs are found at this link: http://pcwin.com/software/Block_List/index-1.htm
To clear history and other traces, check out this Firefox add-on: https://addons.mozilla.org/en-US/firefox/addon/1280

Yes, you'd best get another system account for your sister. If you're that reluctant to do that, then you have other options to choose from.
You could download another browser and use that one or have your sis use that one instead, although this probably won't work.
Use MSN Explorer. The signup process is tedious, and I personally adore Mozilla, but MSN Explorer requires that you log into an account to be able to use it. Because of that, you'll be able to differentiate between who accessed what site.
As corny as this may be, set allotted times for using the comp. Unfortunately the last person can stay on for as long as he/she is one, so it's not that good.

Even if you block it, the URL will still go in the history. She just won't see the site.

Installing a web filter will help if she is not very good at getting around them. This one is both good and free:
