If he is holding it close to his body for long periods of time, yes, he is sick. Puffers will hold their tails straight back the majority of the time and have fins held out away from their bodies when they feel normal. Test the pH and ammonia levels in your water. Also check him over for external parasites and fungus and treat accordingly. Regular water changes are very important for the health and vitality of your tank. If you have fallen behind change 1/10 of the water each day for a week and be sure to add dechlorinator and stress coat to the new water. Good luck!

Puffers have larger bodies and use their fins to swim differently than most other fish with which you might be familiar. The tail is used for both swimming and steering (along with the paired side fins).

Your fish might not have a problem other than your filter is producing a current that's too strong for him to swim easily. So all he can do is to use the tail like a boat rudder to keep his position while paddling with his side fins. Try slowing the flow rate on your filter (there should be a knob at the top to let you do this, of if your filter is an AquaClear, the whole tube turns to the side) and see if his tail position changes.

Mine do the same thing and I have had them for about 6 months seems normal to me
