I have arraged the meeting with my boss and an HR rep.

Ask them if they have a second to talk to you and take them into your office or a conference room and let them know what you have set up, who will attend, and when it will occur. Be sure to offer them a copy of their goals or performance plan if that is a tool your company uses. Its best to do it in person and ask them if they have any questions beforehand about how it will work and what it will entail.

Send an email or write him a memo, " Joe, we will have a meeting with --- on Thursday, Dec 13 at 10:00 am in ------- conference room. Please arrange your schedule so that you will be able to attend.""

That's all you have to do.

send a meeting notice to the employee / subject expectations. keep it simple.
