what happens during a breakdown?

You usually don't know at the time. Those experiencing a 'breakdown' are no longer able to function normally in their every day lives. They may be so depressed that they can't make the effort to get out of bed; they may have severe and unpredictable mood or behavioural swings; they may loose touch with reality and hear voices or experience panic attacks.
Most people recover either with the help of medication or therapy.

if you had one, the people around you would have no doubt. Though you yourself may still be too numb to know for sure.

The actual "break down" may change from person to person.. but usually has a level of high anxiety followed by uncontrolled emotions and sporadic behavior... then on to a "down" period. At least that's how I'd describe it... you can probably get a better explanation on WebMD or a site like that.
