If it is an electric water heater it sounds like an element has burned out assuming you have two elements and it's usually the bottom one that sits in the mineral build up. this can be tricky in replacing as it takes a long time to drain down the water heater due to the sediment. Unless you are pretty handy I'd call appliance repair, still cheaper in most areas than a new water heater.

If your water heater is electric, I would say that one of the heating elements is bad. If it is gas, it could be a bad dip-tube. The dip-tube takes the cold incoming wtare to the bottom of the tank and pushes the hot water out. If it is broken, you will be getting the new cold water off the top of the tank.

It could be the heating element or it could be your heater is not set hot enough. If you are using almost exclusively hot water, not mixing the hot and the cold as it is designed to do, you will run out of hot in no time.
