She needs to be always testing first thing in the morning to get the correct dose of medication. Also on her current medication, she needs the first thing in the morning TSH around 1.0, otherwise weight loss will be very difficult. She also might want to make sure she is converting enough T4 into T3. She can tell this by having a free t3 test. It should be at least midrange. If it isn't, then a T4/T3 med might help.

As far as diet, she needs to increase fiber and avoid soy. Exercise should be done at least 4 times a week.

I too have Hypothyroidism. Losing weight is a challenge.
But the exercise such as walking a mile or two daily is important to help with it.

Eating light low calorie meals and exercising will help a lot.
But it has to be day in and day out. Two pounds a week is the normal weight loss. So 40 lbs. will take about 20 weeks or 5 months, then she just has to stay on top of it and stay active.

The older you get the more difficult it is to lose.
