A doctor (in the US) must get an MD. Your undergraduate work should be related to science (especially chemistry and biology), although technically you can major in anything. There is a rigorous entry test, the MCAT for medical school which is another four years (so you are about 25). Then you must intern for one year and usually enter a residency program to specialize (depends on the specialty--one to six more years). You are working but not making the big money yet. Plastic surgery probably pays the most, but you may not like surgery. Pediatricians and general practitioners make the least, but they are happier in their field than other doctors.

MCATs are difficult, especially if you did not major in chemistry and/or biology. You can get an idea of MCAT questions by looking at a study guide in the bookstore. You can study out of the country but you have to pass the board exams which can be difficult if you didn't get a good education. However, I know of doctors from Caribbean schools who did well -- you still do your residency in the US.
