Healthy? Prob not...the reality is you can love and not trust...the two can be independent...in regards to this situation he prob got hurt really bad at some point by a female and may not be fuly healed....thus the trust issues. He is prob capable of trusting again he just needs time and to be shown how with love and support.

People have radically differing notions of what "love" is, so in some limited context it's possible that he loves her yet doesn't trust her. But in my understanding and approach to loving and being loved, love doesn't exist apart from trust.
That said, it's not my place to evaluate whether this arrangement is healthy for them or not...if this works for them, all is well. Time will tell for sure.

No it is not healthy. If you are in a relationship and say you are in love you must also have trust in the person you say you love

You can absolutely feel love for someone... trust and love should go hand in hand for a relationship to work... but sometimes the trust comes later.
There are two type of people:
-Those who trust indefinitely and automatically until trust is broken
-Those who believe trust needs to be earned
In this case, I have never trusted anyone fully and completely - for me, trust needs to be earned. That does not mean I wasnt completely in love with someone... it just means that before I allow myself to become completely vulnerable, I need to be sure they will take care of me.. meaning take care of my emotions, my heart, my fears, my dreams... all of it.
Others trust easily... and though I envy those that do, I definitely know how to protect my heart better than those that do.
It takes time. You cannot love someone entirely until you know who they truly are.... and with that, comes trust.

It's not healthy. Tell your sister she needs to get out now. Tell her to run fast. Tell her to not look back. He might be gaining on her.
