Generally speaking, by proving you're smarter than the people that teach at the particular institution. Once you realize that the "honor" in the "honorary degree" is for the institution that can now use that person's name in connection with theirs, not for the person who gets the degree, it all becomes a lot clearer.

Usually, by having a job that makes you important to lots of people (politician, movie star, etc.), and then making a large donation to the college/university in question.
It usually only happens after having a lifetime of success without the degree.

Either by being famous, so the university want to be connected with you, or by being connected with the university anyway (they're often given to long-serving academics when they retire)

You don't earn one. You get it because someone thinks that you are worth it. It may be thanks for a contribution, appreciation for excellence in your field, or just because someone likes you personally.

most (if not all degrees) start as honary eg MA (hons) BA (hons) BEng (hons) MEng (hons) etc.
if you are deemed to have failed a module (universitys own guidelines so it does vary) you will be dropped to a BA or BEng etc with no honours.
