Baseball Reference (once again) provides the goods. Each team-season page has a link to all transactions for the year (01-Nov through the second 01-April, encompassing the playing season and the preceding and following offseasons, so there's overlap year to year but you won't miss anything).

And you can go to other years easily.

However, "good/bad" assessments are left to the reader.

Fred McGriff for Dale Murray is one that comes to mind. This was before The Crime Dog even made his MLB debut. Poor scouting on the Yanks part. He was a keeper.
