It's expected that a child might create an imaginary friend for a finite period, and children are generally not affected by schizophrenia. An adult, on the other hand, ususally doesn't have imaginary friends, and when they do, it's usually due to a life-long mental or personality disorder. The big difference, however, is an adult with schizophrenia is unable to discern reality from imagination or hallucination, and their imaginary friend(s) are often coupled with demonic voices in their head that instruct them to behave anti-socially. Also, Schizophrenia isn't defined as having multiple personalities. In fact, it's not common for a person with schizophrenia to have multiple personalities, but it is common people with multiple personalities to have schizophrenia.
