What ozark said is true and you are very likely looking at buying a new jacket, however, if you are determined to try and save it take it to your nearest saddle store or western store that sells tack for horses. They are leather experts ( or know where to find one ) and have all kinds of soaps, oils, glycerin bars etc to care for and rescue leather. Leather saddles, harnesses etc take a beating in the horse world with dirt, sweat, rain and manure so these people know how to care for leather.

By the time leather is dry and crackly looking it is probably too late to do too much about improving its looks. Like any other material, especially non-synthetic, it loses its ability to hold together over time.
The only suggestions I can offer are to find a good quality leather treatment product and start applying it in small portions to your jacket. You could improve the moisture content. However, if it has flaked off, there is nothing to restore, since the underlayer is not finished or processed.
By the way, I have had similar experiences with the leather on car seats. The final solution was to cover them with something else, or buy new leather for it, which never matches the rest of the leather, say, in the back seat. Maybe it is time to find another coat, this time treating it on a routine basis to keep it pliable.
