Noone could give you a definite answer.
Some will tell you it was just a typing mistake that wasn't bothered with to be corrected, and became popular.
Others will say it originated as a typing trend involving incorrect grammar and spelling
Example: "eye iz goin ta pwnage u lul!" or "iz is teh pwnage"

a second cousin is where your cousin had a baby. A cousin once removed it where you are related to someone but they divorce the wife and you belong now on the wifes side. so that makes the wife your cousin once removed because she is related by marriage.
