the middle two seem right. you could also say "low income senior citizens are not covered under Medicare Part-D", emphasizing their low income.

I don't understand the 2nd part of your question,but the answer is NO it is not right, by virtue of the fact that it is a DISGRACE that this country does not have a National Health Care system similar to France,Canada, Norway, the UK and dozens of other countries around the world. If we did then low income seniors would be covered by whatever the system is called.

Try "with low incomes", since each of them has a separate income.
