A duvet is basically a comforter that needs a cover, a duvet cover, over it. You can use a duvet on its own, but if you spill something on it or it just needs a washing you'll be sorry you didn't have a duvet cover on it.

Same thing, in Europe they are called Duvets and here they are called both, they both need a cover and you will find the covers called by both Duvet cover or Comforter cover depending on the brand.
You can buy comforters here in the states that are already decorated and do not require a cover however.

Not 100% sure but I believe a duvet is used without a top sheet and a comfort goes on top of a bottom and top sheet. I remember years ago in Paris there was a comforter type of bedcovering and it seemed like a comforter with a sheet or thin material covering. The point was that you could easily wash the covering with washing a whole big blanket thingy. I hope that helps.

A duvet is a plain white "pillow case" stuffed with either feathers, down, or some poly stuffing. A duvet cover is simply a decorative, protective "pillow case" for it. A comforter is a pretty, tufted thick blanket for the bed. That is, what I would call, a comforter. That's all you need.

A comforter is normally a thicker, heavy spread on your bed a duvet normally is used over a feather tick or a comforter to make it look a little nicer or perhaps to change style and color
