Anything that can interfere with kidney function. This includes poor water quality, internal infections, overuse (and/or overdose) of chemicals and medications, tumors, and genetic defects. That's one of the reasons why dropsy is so difficult to treat - you need to figure out and eliminate potential causes before you can treat this effectively, and depending on the cause, there may not be an effective treatment.

Treat the dropsy with Maracyn2. It's the only medication I've seen that specifically treats it, and it works! You can get it at petsmart or petco. Walmart doesn't have it. Start it ASAP, dropsy is deadly if not treated soon enough . Maracyn2 also treats alot of other diseases/illnesses in fish, even more than is described on the package. If you just used the bactopur recently, do a partial water change before changing the medication. If you think it needs medication for other problems, Maracyn2 can also be used simultaneously with Maracyn, if you think it needs a different kind as well. They are both antibiotics, just different kinds. If you think the fish is constipated, feed it a couple of blanched frozen pes with the skin removed, and mashed up a little bit. that will help it poo. Good luck, I hope he makes it!

Copperhead again has a great answer, dropsy, by a lot of people, is thought to be just caused by bad water quallity, but it can also be caused by bad diet, to much salt, internal infections, canserous tumors, and anythng that affects the internal organs. It is very difficult to treat and when you see the signs of it, most of the time it is too late.
