The lock washer should always be sandwiched between the head of the bolt and the flat washer. Then nut goes on after this.
EDIT - yes sorry, i was confused, it should go head of the bolt, flat washer, material being bolted, lock washer, nut.

lock washers are generally for nuts and bolts and right under the nut. With screws the lock washer is unnecessary as the threads hold the screw tight, you can still use a flat washer with a screw to keep the head from sinking to deep.

typically flat-lock-nut is the order. the flat washer protects the material. the lock washer hols the nut. a lock washer will work better 'locking' if no flat washer is used.
