I'm not sure that is necessarily racist, it would be a bit like insisting on calling someone Deborah when their name was actually Debbie, for example. It is, however, totally out of order. The person in question has every right to insist on being called by their proper name or their own choice of nickname. To insist on calling someone another name is humiliating, inconsiderate, and deeply wrong, as a name is very personal.
The person who made this comment is totally out of order and should be given a disciplinary over this treatment of the service user.
Be careful about bringing 'race' into it, it could be that the carer's intentions are racist, but on the other hand, she might just be arrogant and inconsiderate.

another very worrying point is the last sentence "you live here now we can call you what we want" what is this "carer"implying?Is there no training in this workplace?What other methods can be used to dis empower this unfortunate service user?
