When you win an item on Ebay and the users allows to get pay via Paypal it will tell you that you have WON and now need to pay for the item.
Once you click the Pay now icon it will take you to the Paypal page and here you would agreed to pay for the item.
The seller then will get a payment confirmation and hopefully will ship your item
