i would say if this trouble was threatening/could threaten the life of someone else, other then the person in trouble, then yes you have a responsibility to tell someone, if it is threatening just the life of the person in trouble, then you must decide if you care, whether about that person personally or about that person as a human being, if you care, you tell, otherwise you must face the fact that you dont care,
there are ways to tell without revealing your identity, if that is a concern
i think for non-life threatening things, you must look at each situation,

That depends on the kind of trouble, and their age and if they are competent to make decisions and if their actions affect others and how.
