North Star and Blonde B, you two must have been at the same dinner.

JuJu good one, removing your hat. My father used to do this and I admired that. Same with removing it for prayer or other respectful doings.

The ones that I was going to say have already been said.
Blowing the nose
Passing gas
Burping
Chewing the food with the mouth open
Talking with food in your mouth
Smacking your lips
Coughing and or sneezing without turning you head away and covering your mouth.
Not saying please and thank you when asking to pass you a certain dish.
Here are a couple no one has said that I recall.
Using their own spoon or fork that they have already had in their mouth to get more food out of a dish to add to their plate.
I'm sure some one will get offended by this one, but I can't stand it and have seen it many times. Letting their pet eat at the table with them.
Putting their ashes from the cigarette/cigar in their plate after they are done eating and then when they are done with the cigarette/cigar they put it out in the plate.

Years ago a lady would sit at the table and then take off her gloves. A gentleman would take off his gloves and then sit down to dine. It's been going downhill ever since that practice went out of fashion. Mouth open while eating, talking while eating, is never done. Knife and fork together on one side of the plate when you've finished. Simple things make for a pleasant dining experience for both. I blame his mum and dad, probably pigs an' all.

putting elbows on the table
talking with food in mouth
reaching for dishes instead of asking that they be passed
not placing your napkin in your lap
cutting up all your meat at once so it looks like kibble
refusing a dish and telling your host (or whomever did the cooking), "No! I hate that stuff!" (give a nice excuse, instead)
blowing your nose
talking loudly
gossiping or being very negative in your conversation
leaving the table as soon as you finish eating
not complimenting or at least thanking the cook
leaving the table without offering to help clear and wash up
leaving a lot of food on your plate

Definitely no hats at the table! Or indoors, for that matter. I am nauseated by people who load their forks so high that they can't chew the food with their mouths closed. And then continue their conversation! Or people who make layered arrangements of all the food on the plate onto their fork. Or people who crush huge amounts of crackers into their soup and eat the sludge. Or people who put the bones or pits on the table. Or the person who didn't provide a dish for people to put bones or pits into!

Years ago men removed their hat before eating. Looking across the restaurants now a days it's a sea of baseball caps. Oh well, things are a changin'.

Talking with your knife and fork in your hands. Once you have put the food in your mouth, you should put your knife and fork back on your plate.
Not putting a serviette on your lap.
Talking with food in your mouth.
Swallowing your food down with your wine.
Flapping your elbows out, your elbows should stay at your side, and putting your elbows on the table.
Talking with your mouth full. Chewing with your front teeth. Chewing with your mouth open.
Not standing up for a lady, when she leaves and returns to the table.
Eating your soup with the spoon toward you. You should use the spoon in the direction away from you.
Not placing the knife and fork down in the correct position, when you have finished eating. These are the things that dirve me mad. Or make me cringe.

Shoveling the food in like it's going out of style. Feeding your face more than your mouth. Tucking your napkin into your shirt collar at a classy restaurant. Nonstop talking during the meal. When on a date with a lovely lady you give her the check :0!

The following were punishable offences when I was a child and they still make me cringe today.....elbows on the table, talking with food in mouth, licking the knife, slurping soup or drinks, using fingers to pick food up especially when it's not a finger meal, not saying please and thank you when asking for something to be passed across, not saying thank you to the person who cooked the meal, swinging on the chair,

In addition to the afore stated nasties -- it's terribly bad manners to gnaw on the communal bones out of turn.
