My wife and I always comment on that issue also. We have experienced it from the perspective of being our car, in Tokyo traffic. I have had an ambulance come from behind with it's lights and siren on. With the way traffic is and the narrow roads, with no room at all, there is no way any other car can really get out of the way. To move forward in to the cross traffic is a danger and another tradgedy, the sides are often lined with a barrier to protect the pedestrians. It sucks for the emergency vehicles and the people who require help. I have noticed that if there is any chance of moving out of the way, drivers who can move do move out of the way. In general there is always a sense of cooperation and courtesy on the roads for everyone. I have also noticed that the ambulances shut down the sirens when they get to an intersection where they cannot pass or find a way through.

Yes and it worries me because a lot of people will not let you pull over to get out of the way!

No, I just automatically pull off to the side of the road and wait until it passes, then go on my way.

No, I'm good in emergencies. I just calmly pull off to the side as soon as possible.

No. I just try to find them so I can see which direction they are in/headed so I can get out of the way.

If you panic you shouldn't be driving. These are perfectly normal sounds in the city.
