I dont remember anything else about this movie except this.

there is a husband a wife and a kid and they live in a house at the bottom of a hill. the husband always takes the car out of gear at the top of the hill and lets it cruise down into the driveway and the wife always says "don't do that you'll kill someone"

then at the end of the movie the husband does it for a last time and he ends up smashing into the back of a car -

inside the car is his wife and some guy she is cheating with and she is performing orally on this guy when she dies.....

anyone know?

The World According to Garp. I don't think he coasts down the hill, but I do remember he turns off the headlights while driving at night. His wife doesn't die. One of the children in the car does. He talks to his wife about this in the hospital because one of them mentions that he bit the guy's thing off while giving him head.

Actually you're talking about "The World According to Garp" starring Robin Williams, the wife doesnt die though, he (Robin's) breaks his neck, the boyfriend loses some "precious" inches, and their child who was in the backseat gets pitched forward onto the handleless stickshift and loses his eye.
Its a brutal scene but one that rings so true about karma and fate and doing wrong. The wife is a teacher and the guy is one of her students. Its a really good movie and made John Lithgow into a star. He portrays an ex NFL football star who becomes a woman, he and Robin have some really great scenes together. They develop a really wonderful relationship where robin's character accepts John's character as a woman, albeit a very muscular woman who can still kick *** when she needs to. John's character is particularly helpful and supportive of Robin's character when the accident happens and he is a very bitter broken man who feels like he failed in life. The book is even more wonderful you should read it.
