Depends. If there needs to be a scene set behind it while another is play in front some directors choose to do that, but normally just open at the start, close for intermission, open after intermission and then close at the end.

It all depends. Sometimes just a dimming of the lights and then bringing them up again will indicate a scene change. The curtain should close any time the crew has to work on stage and for intermissions. Many theatres do not have curtains and get along quite well with just lights for act and scene changes.
