You can only park in this type of space if you meet at least one of the following:
A. YOU are disabled - you ask your doctor for a prescription for handicap parking - written on his prescription pad - take that to your DMV or similar agency - they will also probably want you to fill out a form and also have your doctor fill it out - take it all back - possibly pay a small fee and you get either a permanent plate or tag - or a temp tag.
B. You are driving someone that is disabled and they already have a perm. or temp tag - you CAN use it as long as they are with you in the car.
As far as the fees, and forms and prescriptions go, yes this info is correct - I live in AZ and had to go this route for my mom- but, I did not have to pay any cash in Arizona- this was nine years ago though.
Now in Texas - I just did this in October 2007 for my grandmother - yes to all including the payment - I obtained two because my grandfather has two vehicles - and the cost was $4 for each one - good for 4 years and then she'll have to renew.

Do not get me started.
A bloke who uses our local shops is disabled, and I have every sympathy for him and all like him (wheelchair, not old, 40's maybe), but he is sooo rude.
Bumps into you with his wheelchair and shouts ''excuse me'' even if there is lots of space around you, puts in no effort at the counter when being served, but expects everyone to wait on him hand and foot.
Sod him, he gets the best parking spaces, shops have spent thousands to give him better access, and he is perfectly able bodied above the waist but will do nothing for himself.
He also gets the best damn seats at the football ground and any other public event.
I could live with all that if he was only as polite as the rest of us, but no, he needs to be special.
I would not want to be in his shoes (or chair) ever, but please, that does not mean you are now above everyone else and can't be polite at the very least.
Take his parking space.

In our area of the U.S. you have to ask your doctor for the form to apply for a disabled parking permit. He/she has to fill out a portion of the form and they will not do so if you don't have a real reason to get a permit as described by law. Then you take the form to the motor vehicles department where they issue the permit and hang tag or license plate---which ever you prefer.

you have to have a tag to hang off your rear view mirror or a handicapped license plate. those can be obtained from the license branch with a medical statement from your dr stating why you need one (or why someone in your home needs one). if you aren't truly disabled, leave those spaces for those who are instead of being lazy.

go to the dnv they will give u a form fill it out and tell your doctor to sign it to prove your disabled and go back to the dnv and turn it in they will give u a temporary one until the official comes through mail.

IF YOURE DISABLED THEN THEY GIVE YOU A STICKER OR THE MIRROR THING AND THEN YOU PUT IT ON YOUR CAR...THEN THE POLICE WONT GIVE YOU A TICKET=] sorry it was in caps
