Just write it as if you were talking to the person without letting any bitterness show so that if needed, you can get a recommendation for the next job. for example:
I would like to tender my resignation from your company as of ( whatever date you choose) for personal reasons and because I think I can no longer be a good employee at the present time. Thank you for giving me a chance and I am sorry I now find the need to leave. Sincerly,

I can understand how you feel. I would recommend first, seeing a doctor for your anxiety. Second, is there an HR person or manager you can talk to about your concerns and stress? Let me tell you first hand how tough the job market is right now. Good jobs are really hard to find, including decent pay. If you quit your job, your chances of getting unemployment are very, very small - unless you can 'prove' hostile work environment and that takes time. You will also lose your benefits and COBRA costs a fortune. So please think about these things before you make a decision like this. Writing a letter as you suggest will be kept on file for a long time, and you may serious regrets doing this, down the road. I wish you good luck in making your decision.

There are several issues to consider here. One is that the economy is terrible and jobs are very, very scarce. That being said, I do know what it is like to have the issues around a job make you feel a little nuts. DO NOT write a letter to send "to everybody in the company." THAT is the sign of a disgruntled employee. If you do leave then allow yourself time to unwind some and then address a letter to only the people you directly answered to, perhaps the upper management if absolutely necessary. Respectfully address the issues without placing blame. Lastly, don't expect that the one person who caused you the most misery would ever see those things in herself. She would most likely never accept that she has caused any problems.

I know you are not happy with the situation you are working in....However, quiting job you may loose all of your benefits that you are entitle for. You should quit when you have found a different job.

You can simply approach the person who hire you and say you quit. You may not need a formal letter.

Basically... keep it short and sweet. A letter of resignation is NOT the place to vocalize your issues with the company!

Dear Management,

Due to (scheduling conflicts, money conflicts, finding another place of employement), I am submitting my formal resignation. My last date of employement will be . Thank you for your understanding of the situation. I apologize for any inconvience.

dont forget to date it! And try, if you can, to submit the resignation at least 2 weeks before your last day. tah-da!

TO WHOM THIS MAY CONCERN:just keep it short and don't say anything about the company maybe you could just say because of personal reasons make sure you date the letter and sign your name (be professional)
