The needle needs to go into your glutes (the part of the butt you sit on) so only lowering them to the top of the leg is ok.
This is done by a doctor, they've seen TONS of naked bodies, nothing really bothers them anymore. No need to feel scared or ashamed. The first time I had a pap smear done I was really nervous but they were professional about it. It's nothing to have an annual physical anymore.
I'm not sure if it'll hurt more than a flu shot, but you'll feel something. It won't hurt for long.

You should not have to remove your pant completely. And you should definately be able to keep your panties on, you may just have to adjust them depending on the site. There are two spots where he can give the shot, the most common one is on the side just a little back and lower than where you can feel your hip bone. It is called the ventrogluteal. This site is preferred by most doctors and nurses. The other site (called the dorsogluteal) is close to the middle of the cheek and slightly higher, but this site is not often used anymore. It really depends on what kind of shot you are getting. If you are really embarassed, ask if it is possible to have the shot in the arm. Also keep in mind if the Dr. says he wants your pants removed, you should be able to ask to have someone stay with you or have a nurse with you in the room. It's your right!

It is not required or necessary but just makes the doctors job easier
You should not feel ashamed at all, and I know thats easy for me to say, I work with patients all day every day, I've seen it all
It's not really that painful
If it's a male doctor he will almost always have a female nurse in the room to avoid lawsuits and to make the patient more comfortable, this should help a little bit
Do not be ashamed
I'm 18 and just started a battle with breast cancer, I've had all kinds of male doctors (that I work with) feeling me up. You just have to remember, they could care less and aren't lookin at you that way, they're just getting their work done

not really on your butt. more like you upper butt. he probably wont even see you crack. All you have to do is pull you pants down slightly. No big deal. All shots hurt a little but it won't kill you.

If the Dr doesn't ask you if you want a female nurse present, request one...but I think he will. It's pretty common.
