The wireless gateway has to connect to the internet, so yes there will be monthly fees for the internet connection.

Most of these connect to a DSL line and then supply wireless within your home. Yes they work well with the Xbox. You can also run your computers off the same network.

If you already have DSL there won't be any additional fees.

Added: Since you already have DSL, just adding a wireless router costs you whatever the router costs, thats it, no additional fees. If you have SBC and Have a 2wire Modem it is a router and has wireless.
If you have a SpeedStream modem you may just want to call SBC and switch to the 2Wire unit. (It is great, have one here!)
Has four computer ports and wireless all in the "modem/router" unit. You can also just get a Linksys, or Dlink , Netgear, Buffalo, or any other wireless router from your local store and setup wireless. Its really just all in the router and any Cable/DSL wireless router will add wireless for your home.
