Maybe because they don't trust that their marriage will work out and fear that their spuose/partner will do it anyway, so they give permission because it won't hurt as much if they allow them to do it with someone else rather than it being done behind their back. Or maybe they feel it's not 'cheating' if permission is given.

Because they have sex for a living (porn, legal prostitution)
Because they are polygamists
Because they have an open/casual relationship with someone they're not in love with
Because they're just friends with benefits

Because that's the kind of relationship they want to have. It takes a very strong people, with clear definitions of what they want to be able to survive and open marriage.
