I have been asked to provide a reference for my housemate who is moving shortly, what information should be on there?

-the dates you lived together ("June 2005 to Dec 2007", for example),

Referrence for what? If it's for a new place to live, then the referrence should state how long they lived with you, that they paid their rent on time, and probably that they had no problems with the rules of your home.

Information on her cleanliness (regarding her housekeeping), her character (whether or not she'll trash the place), how long you spent with her, how loud she is at all hours of the night... etc...
