The annual report forms should be on your Secretary of State's web site.

Having studied Accounting in college AND having my own service business ... I can answer this!

Really, the annual report for a new business is mere formality.

Search for "business annual report template" in Yahoo or any search engine and you should find at least a few to use!

Just copy the template and in the formal language, basically say what you just asked. State the official date of the start of the company, the current assets (furniture, office supplies, etc.) and any capital that you have (money in the bank).

State that the company is in the process of acquiring customers and is in an "exploratory" phase. Or some such language, as the official document will only be used when you want to go to a bank or the SBA to apply for a business loan.

The annual report is done by your accounting firm after their yearly audit and usually sent to your stockholders. If your company has not done anything yet, I would not send out an annual report since there is really nothing to report.
