I haven't read many, but so far number 1 for me is Cell. It's mostly the beginning, but there are occasional blood and guts parts later on. I can't really number any more because the ones I've read are pretty darn "clean" compared to that one.

I found the scene where Doloress husband was trying to get out of the well quite disturbing. It's the only part in any of the books I read that I found to be "scary"
