Part of the problem could be , because you are too concerned on what is proper & what is not!
Improvement in grammar is similar to improvement in skills such as gymnastics the more one practices the gymnastic moves together as in a still ring routine or balance beam exercise the smoother they come together.
--Ultimate perfection in gymnastics everytime in each apparatus(which could be compared to different types of conversations you have)is pretty impossible , thus gymnasts strive to improve in each competition!
--Like wise , if you are speaking with others the best you can(and doing what you can to improve)and not overly concerned with reaching perfection each time , you will indeed be improving, and you will not be messing up!
You are an individual , and have your style of speaking , if you want to be like someone else, you will stiffle yourself and find yourself stumbling!
Relax and enjoy any conversations you have with people , that is when your grammar will improve and you will find pleasure in speaking!

Maybe it's just a matter of being too self-conscious. Relax and be yourself! Remember, the other person is probably just as absorbed with his own worries and most likely doesn't even notice your "mistakes".
