I think everyone would love Elfen Lied if it was on in American TV, but a little violent(ok really violent). I think this show should be respected more.

An absolute masterpeice. Each episode tells a different story. It's a fantasy/drama unlike any you'll ever find. my favorite anime.
Mushi are neither plants nor animals and their existence and form are unknown by many. Ginko, a Mushi-shi, travels around to uncover the mysteries of the Mushi. As he investigates, he helps people who struggle with problems caused by these mysterious creatures.
This anime has some of the most beautiful artwork I have ever seen. Although it's a fantasy and there isn't any character development because of the way it is structured, everything is done so well. You come to care about the people who are affected by these mushi, even though they only appear for a single episode.
The music is superb. Very serene; it is a perfect fit for the extremely high quality animation throughout the entire series. I listen to the OST everyday.
I really wish more people would watch this anime. It is something which should be seen by everyone. I admit that I put off watching it at first. I think people are always looking for animes that have a huge amount of action. It's a shame because there are so many different animes out there that work just fine without it (Death Note for example).

Although it ended early (the manga was never finished), this is deffinitely worth watching. If you sit through the first couple of episodes you will be surprised. What seems like a typical story at first turns out to be far from typical. Some of the best character development I've ever seen in anime.
Juuni Kokki is a fantasy. It's about a girl who is suddenly forced into a position she doesn't want to be in, but she doesn't have a choice. She has to learn about her situation and become stronger. I won't explain in much detail, but this one should not be missed.
The Twelve Kingdoms isn't as unknown as some other animes. It's actually considered to be somewhat of a classic, but that is to those who have seen it.
Although I actually enjoy Bleach and Naruto, I believe they should not be more talked about/popular than many other animes out there. There are so many that deserve it more.
I guess you might say, the best anime is the one that you have yet to see

Avatar and Death Note are pretty popular though, and almost anyone who is an animation fan has heard of and probably watched some of them.
For me the most unheard of and underapreciated show would be Baccano! It was my absolute favorite show this year- and no one talks about it. However, everyone who does watch it rates it EXTREMELY well. Heck, Baccano! has moved into the top 25 all time anime on MAL already. And it deserves that spot. It is short and sweet with some of the best characters that I have ever seen.
As far as ongoing stuff, I'd say Kaiji is the most underwatched. Epic rock/paper/scissors dules FTW. That show is made of win.
It's also nice to see another person here who isn't lured by the mainstream anime of Bleach, Naruto, Inuyasha and FMA. I like people who have experienced more anime than just CN' AS. I think you'll like Bob, too. You can see how much he loves mainstream anime, already XD.

The first anime that always comes to mind when I think of underrated anime is Android Kikaider. I mean, it had action, romance and comedy. The story was also more intellectual than say Inuyasha or Naruto. I don't get why it's not that popular.
I would also have to say Zoids, I was so addicted to that when it was CN. I can't believe that when I do searches on it, the toy pops up and I can barely find anything on the anime.
I mean, those are anime that people have heard of, but they're just so underrated anyway.

Full Moon wo Sagashite, Mermaid Melody, Ouran High School Host Club, and Tokyo Mew Mew are REALLY good animes! You get very into it! They are my all time favorite animes!
Here are descriptions of the shows!
Full Moon wo Sagashite: A twelve year old girl named Mitsuki dreams of becoming a famous singer but she has throat cancer which makes that dream impossible. But when two death spirits named Takuto & Meroko come and tell her she has one year to live she asks Takuto to do something for her. To turn her into a sixteen year old so she can pursue her dream. In her sixteen year old body Mitsuki is healthy and she becomes the famous singer Full Moon.
Mermaid Melody: The Pink Pearl Mermaid Princess Luchia must go to the human world to find her sacred pearl she gave to a boy she met 7 years ago. After getting her pearl back she must find the 6 other mermaid princesses to help reawaken Aqua Regina so she can save the undersea world and the human world from the evil Gaito.
Ouran HighSchool Host Club: A girl named Haruhi gets a scholorship to the prestigious Ouran private High School. Since she is a commaner she cannot afford the school uniform and resorts to wearing slacks & polo shorts. Combined with the fact that she has short tousled hair and large glasses she is mistaken for a boy. Looking for a quiet place to study Haruhi walks into the Third Music Room only to find the Ouran Host Club. While in the room Haruhi accidentaly knocks over a vase worth $80,000 so the Host Club (Thinking she is a boy) make her a host to pay off her debt. Haruhi's real gender is revealed to all the members of the Host Club but she is allowed to continue charading as a boy to be a host as long as she dosn't reveal it to the customers.
Tokyo Mew Mew: 5 ordinary girls get infused with the DNA of 5 endangered animals. They must use their new powers to defend the Earth from the aliens that are trying to take Earth as their own.

I'd vote for Azumanga Daioh. I've watched a great many series, most of which are cookie cutter similar, and the characters quickly fade. But that's definitely not the case for AD. It's the only series, besides Love Hina, I've watched more than twice.

Kino's Journey is a beautiful little show based on a series of connected short stories by Keiichi Sigsawa. It is very subdued and subtle, but every episode will make you think. Unfortunately it is criminally underappreciated since it has no big-breasted robot girls with guns, ninjas or mecha.

hungry heart wild stiker (soccer anime)
this anime is about this guy who quit soccer couse he was always being compared to his brother since his brother is the best soccer player in japan till he is conviced by the girls soccer team captain to play since he has great striking ability(kicking)hes pretty funny and the character really progresses through out the story and has some family drama aswell and some chemistry between him and the girls soccer team also the guys pretty funny almost like goku a little slow
hajime no ippo (boxing)
this one is about ippo makunochi he was a kid who was always being bullied till he was save by a pro boxer and decided to stop being weak and worked on getting stonger the boxer saw his potential and took him to the gyms coach he gets into a rivalry with another boxer his age and weight class and it becomes his goal to box his rival as a pro boxer he goes throgh alot of training but due to some circumstances he isn able to face him so they agree to fight each other in a fight to decide japans champion it helps that the guy has the most powerful punch in is weight class to climb the top of the ranks also it is really afunny anime couse of the other boxers like takamura the strongest boxer in the gym and eventual wmc(world middleweight champion)

I think Elfin Lied is what you are looking for.
It has some gore but the characters are well done and the plot is excellent.
