I tore my rotator cuff while I was off work and the next day i gave my boss the note from the E.R. saying that i could'nt work for a mininum of 2 weeks. He had me fill out paper work to request medical leave from our corporate office, a week later i received a letter in the mail stating that on Nov. 21 I requested medical but I don't qualify for it so as of Nov. 21 I have been terminated from employement. I have worked for this company as a automotive technician for 4 months.

No. Additionally, you were not on medical leave, you were waiting approval for medical leave.

Since you have only worked 4 months you are not an eligible employee under FMLA rules even if your employer is required to provide FMLA benefits.
