I am quite sure that you have no control or ability to save the picture belonging to a contact that you are chating with. My suggestion is that you ask the person to transfer the file over to you via MSN. As long as your contact keeps the same picture you will always see. If they change then you will see something different.

There is no direct way to do it via MSN Messenger, however, just can press PrintScreen button on keyboard and then paste the screen image into graphical editor (Photoshop, Gimp, or even MSPaint included with Windows will work). Then select the picture, crop it and save into file.
