When I was in Japan around 2002 the punk thing was going strong. It certainly wasn't a fashion trend because punk music was also very big at the time so it was either a way of life or the most elaborate and engrossing fashion trend ever. I can't say that they made a very big public disturbance though, at least not by American standards. Japan has always had one of the lowest crime rates in the world and as far as I could tell crime just didn't happen there.

It's like anywhere else: it can be both, depending on the person.
I think you're more likely to find more people in Japan into the fashion aspect, not taking on the whole way-of-life.
The people who disturb the public tend to be yankees. (Not Americans, just a name for a certain type of personality).
Yankees come in a variety of forms and tend to be middle-class kids who want attention. These are the kids that rebel buy dying their hair brown while a student, altering their school uniforms, and getting a scooter when able. Some of them don't have intentions of being delinquents, some are pretty smart, but a lot do need to get attention.
Some become "boso zoku", basically kids that ride around on their scooters all night, with no regard for traffic rules, sometimes egging the police on.
Some step over the line and become chimpira or full-fledge yakuza, the Japanese mafia. Chimpira are wannabe yakuza. Picture shiny white cars with shaded windows. These guys are sometimes brushed off as harmless, but they can actually be quite the idiots, ruining peoples lives. Not usually through violence, but a lot of ruining lives in other ways such as ruining relationships, stealing money, threatening families, etc.
There are too many amazing normal people in Japan to bother with these adult children.

It is a way of life. Some places in Tokyo has crazy amounts of them and they are like street performers, I saw one with all gothic costume on with a karaoke mike siging as he was walking down the street....

i think it is to amuse people. it bothers me but that's because i just got to japan. i don't think it bothers the Japanese. they are all ready used to it. maybe some day it will stop bothering me.
