Never, in any of the book written by Baum, was the name of the witch given. Where the name of the witch came from in "Wicked", I don't know, to be honest.

In Wicked and Son Of A Witch, Gregory Maguire called the Witch of the West Elphaba, and the Witch of the East Nessarose.

She is only called The Wicked Witch of the West in the original books.

In films she has been called Momba and Evillene. In newer books she is called Bastinda and Elphaba.

She was never named in the L. Frank Baum books.

When Wicked came out, Gregory Maguire named her after the original author- L. Frank Baum- LFB- Elphaba.

This info is in the cover sheet of the soundtrack to Wicked- the musical, not the book.
