It's sort of hard for me to think of a horror movie that was scary and at the same time good. I'm a fan of psychological horror, so I'm going to say "The Shining" "Rosemary's Baby" "1408" "It" "Carrie" Some people think these movies laughable, all I know is the first time I saw them they shook me. And if you just want to be grossed out and disgusted and fearful all at once, see "The Hills Have Eyes." Disturbing.

Lots of good, scary movies. One good horror movie I like that I'm sure no one will mention is Let's Scare Jessica to Death, a vampire film from the 70s (though they never really mention vampires in the movie). In the beginning one can't tell whether Jessica is just crazy (she was just released from a mental hospital) or if there is really something out of the ordinary going on. Her voiceover at the beginning of the film and the isolation of the setting really sets the creepy tone. It's not very gory but more of a psychological, creepy type film and it still scares me though I first watched it about 20-25 years ago. It's one of my many favorites.
A couple other of my favs that haven't been mentioned yet are Lost Boys and Fright Night (two vampire flicks with some humor intertwined, and in my opinion some very cute vampires). They probably won't cause you to pass out with fear but they have their moments and are well made and enjoyable. Lost Boys has a cool soundtrack.
For a horror story set in the inner city, check out Candyman. I still can't say his name 5 times in a row (for those who haven't seen the movie, that is the action that causes him to appear); it's a really good urban horror story written by Clive Barker who also did Hellraiser.
The People Under the Stairs is a horror film with a scary basement. It is kind of creepy/kooky horror with some weird undertones-crazy brother and sister (who call each other as mommy and daddy) who lock children up in their home's basement and barricade it from outsiders-I would go out of my way to watch it again.
Dark Night of the Scarecrow used to creep me out but I haven't seen it in a long, long time so I don't know if it still would, but it would probably creep out a first time viewer.
The Ginger Snaps Werewolf movies are pretty good. I discovered them on my local library's shelves and had never heard of them before. I enjoyed them and would watch them over again, and I wish they would make more.
'Amelia', a short film on the Trilogy of Terror (Richard Matheson is the writer, also did I am Legend) is a good one. Before Chucky was even thought about, this ugly Zuni Voodoo doll with a big old knife chased it's new owner Amelia around her apartment (and scared the crap out of 12 year old me). Trilogy of Terror 2 has a story that scares me called Bobby but the 2nd voodoo doll story isn't as scary as the 1st one.
Angel Heart is another excellent movie though I don't see it mentioned often enough. Though not strictly the type of slasher movie one has come to expect from the word 'horror', it does has moments of bloodiness. It has a touch of mystery, supernatural and occult, lusty sex, and psychological suspense. You may have to watch it a couple of times to get everything you missed the first time, I did. One of the stars is the gorgeous Lisa Bonet who also played a daughter on the Cosby Show. Also stars Robert Deniro, Mickey Rourke and Charlotte Rampling. Definitely do not read too much about this before watching it. It will be spoiled if you do. It's more exciting to figure out the connections on your own.
I also enjoyed The Serpent and the Rainbow, another occult/voodoo/supernatural type film set in Haiti and deals with a drug that turns people into Zombies. The Craft is about 4 teen girls who are witches, it's enjoyable but not really that scary. Fairuza Balk (one of the witches) also costarred in Waterboy and American History X (not horror genre-except that it's about Neo Nazis) and she has amazingly beautiful eyes. Oh yeah, did anyone mention Pumpkinhead?
There are many more scary ones but others have already or will probably mention them. Unfortunately the scariest movies I've seen, you will never be able to see as they are in my nightmares. I have a doozy about a vampire in a castle tower and one where Zombies invade my apartment. I am probably one of the few people in the world who likes to have nightmares.
Most of these are 10+ years old as I'm sure others will mention more recent ones.
Most important, don't watch a horror film with a non believer (someone who laughs and says 'that's not scary'), always watch one at night with NO lights on, with the windowshades all the way up and the closet doors open, otherwise, what is the point?--but you can keep your feet off the floor and under a blanket because something is under the couch and that's just way too much 'scary'!

Well, if gore does it for you, I'm sure saw would tickle your fancy. But... if you want a good horror film that doesn't need to rely on blood as a crutch I would go with "The Moth Man Prophecies" "Signs" "The Village" "Skeleton Key" to name a few.

can't go wrong with the exorcist and Ju-on the japeneese version of the grudge is good, the new dawn of the dead is good. They really can't do horror movies anymore

I enjoyed the Saw series. Psyche and blood slasher flick all wrapped into one. Touche. ♥ Any Stephen King movies, too, mostly. Those are pretty creepy.

if you get a chance to watch Rings, a 15 minute prequel to ring 2, you won't regret it
