The QB is responsible for getting the ball into the hands of the running back or wide receiver (on an end around or reverse play). Any fumble during the handoff is charged to the QB no matter what the RB/WR is doing.

It actually depends on the exchange. An NFL official in the booth will make a judgement call. If the qb puts the ball in the bread basket it will get charged to the rb. If the qb puts the handoff anywhrere else it will be charged tp the qb.

Its not like baseball where the player gets charged with an error. But both could be to blame, the QB for not planting the ball firmly in the belly of the RB and the RB could be to blame for not grabbing the ball firmly before looking for the running lane.
